How to become a respite/habilitation provider?

Do you have a disabled person in your home?

Would you like to provide services yourself or have a family member you feel would do a great job?

There are two ways to become a respite/Hab provider for Track House Life.

  • 1. Referrals from our agency.
  • 2. Have a person with Developmental Disability in your home.

Orientation for in home respite. (Orientation form)

Before Respite/ Habilitation services can be authorized, the following requirements must be met:

Complete our standard application. Once submitted we will arrange an orientation appointment. A THL staff member will then be notified of the appointment and attend meeting with you (parent/Guardian). THL Agency will meet with your family to obtain necessary information which will assist in the completion of certification/training.

 

Future GSE program

Our GSE program will be expanding to enclude janitoral employment, Start Date: May 2011

 


 

Counseling Services

Track House LIfe & Carebridge Counseling Services provides in home counseling and assessments across the valley.

We have trained professional therapists with certification ranging from LCSW’s, LAC, NASW’s to MSW’s. We are currently contracted with Mercy Care Advantage, Medicare Part B, Arizona Department of Juvenile Service, Child Protection Services, Adult Protection Services, Vocational Rehab Services and Department of Developmental Disabilities. Counseling for DDD consumers must be approved by the ISP team. We provide in home and can set up DTA sessions.

Supported Employment Work Shop Open

For participation Complete application located in the employment section

Group Supported Employment will be available at Track House Life. Hands on job training while on the job. Consumers will receive the specific assistant needed to maintain successfully employment. We offer Support Aid individualized employment sevices. Support Aid will include, personal care services, behavioral supports and one on one coaching needed to maintain employment in the community.

Our individual support plan at Track House Life will hands on job training while on the job. Work opportunities for each individual include several employment opportunities such as distribution, stock work, retail store work, landscaping, janitorial, maintenance work and other suitable employment that can be obtained in the community. Assistance includes, job search and any preparation towards gaining the employment situation and maintaining employment in the public setting. Our Job specialist will begin the process of job development with the consumer analyzing, identifying strengths, barriers and general job related skills needed for a particular work situation. The consumer will assist with the job development process and deciding what work environment they feel best for them.

Transportation is available for locations not exceeding 25 mile of the work location. Consumers will be transported too and from onsite Center Based Employment or group supported employment in the community five days a week, Monday to Friday.
