Track House Life will provide a variety of interventions designed to maximize the functioning of our consumer's daily living. Services identified as Attendant Care Service, will include support for the consumer living in their home, and working in the community also assistance for the consumer to continue or acquire routine community activities. The goal of Track House Life Attendant Care Services is to: assist with daily living activities, supporting the consumer to obtain or maintain a safe and sanitary living environment, maintain personal cleanliness and overall pursuit of a healthily living environment as well as maintaining and supporting and encouraging community activities. Services will be developed according to a general plan of care directed by the consumers ISP, which will be structured in an Attendant Care Agreement, developed upon initial intake for services. Attendant Care Agreement will be completed 3 days after service begins. Basic consumer skills such as shopping, banking, money management, community resources, which includes assistance with community transportation, this service will be included within the consumer's objective. Individual objectives are based on information gathered from the consumer's expressed personal needs and physical supports needs. Assistance with Daily Living Skills will include but not limited to: Bathing, Oral hygiene, Toileting, Bowel and bladder care, Dressing, Shampooing, Ambulation, Transfer to and from Wheelchair and or Bed, eating and meal preparation, routine nail and skin care, tasks necessary for comfort and safety of movement restricted consumers, assistance with special appliances and or prosthetic devices. Track House life will also provide assistance with meal planning, shopping, storing, and cooking food for nutritional meals. Assistance will include self- medication or reminders. Appropriate attention will be paid to injury and illness, including referring appropriate action to consumer who may present additional medical or social need during time of service provision. Track House Life will also provide assistance to obtain or maintain a safe and sanitary living condition in the consumer's home. Task may include but not limited to: Dusting, cleaning all areas of the consumers home floors, bathrooms, oven, refrigerator, windows, kitchen, washing dishes, changing linens and making beds, washing, drying and folding consumer's laundry( ironing when necessary), and taking out the garbage. Other duties determined appropriate and necessary by the consumer's ISP. Track House Life will also provide additional services in case of unusual circumstance additional tasks may include, washing walls or ceilings and yard work and moving away debris. Track House Life will work with consumer's family, if available to increase and or maintain targeted goal direction.
